1. We are the neighbors/friends of spouses KEN A. DISNEY and BARBIE C. DISNEY, the parents of CINDERELLA CANWOOD DISNEY;

2. Likewise, we personally knew that said CINDERELLA CANWOOD DISNEY is the first daughter of the aforesaid spouses who was born on December 28, 1970 at Barangay Princesa, Nueva Ecija, we, being present during her birth at the behest of his parents who is a very close friend of ours;

3. In fact, when they moved in this city immediately after her birth, we became neighbors up to the present and said CINDERELLA CANWOOD DISNEY, in fact, became the playmate of our children here in Brgy. Bel-Air, Makati City;

4. The foregoing is executed for the purpose of proving that CINDERELLA CANWOOD DISNEY is the lawful daughter of KEN A. DISNEY and BARBIE C. DISNEY, for purposes not contrary to law, morals, good customs and public policy and for other purposes this may serve.

I, MALAKAS AKOH, Filipino, of legal age, married and a resident of 143  ILOVEU St., Barangay May Forever, City of Manila , after being sworn to in accordance with law, depose and state that:

1.      I am married to MAGANDA SIYA-AKOH since July 21, 1980 in the City of Manila;

2.      Out of that marriage, MARIKIT S. AKOH was born on December 28, 1982.  Copy of the Birth Certificate is hereto attached as Attachment 1 and made an integral part of this Affidavit;

3.      Our only child, MARIKIT S. AKOH died last July 31, 2014 at Nagpapagaling Hospital in Manila after giving birth to her 1ST child. Copy of the Death Certificate of MARIKIT S. AKOH is hereto attached as Attachment 2 and made an integral part of this Affidavit;

4.      Sometime in 1983, few months after the said child was born, MAGANDA SIYA-AKOH left the conjugal dwelling and has disappeared from our lives since then;

5.      Our only child never met her mother during her lifetime since we have no personal knowledge whatsoever of the whereabouts of MAGANDA SIYA-AKOH;

6.      Since MAGANDA SIYA-AKOH left our family dwelling in 1983 until as of date, I never saw her again and no information whatsoever of her whereabouts;

7.      Diligent efforts were made to locate said person but no avail.  We have no contact whatsoever from the relatives and friends of MAGANDA SIYA-AKOH;

8.      I   execute   this   affidavit   to   attest   to   the   veracity   of   the   foregoing averments and for any purpose it may serve.

3.      On July 31, 2015, I parked my said car at ABC Mall In Quezon City, to attend a meeting with client;

3.      After more or less, two hours, I went back to the car and found out that the left side mirror and driver’s door were broken/damaged;

4.      I did not witness the incident and exerted my best effort to locate some witness or witnesses as to what happened and why my car was damaged but to no avail;

5.      That I am executing this affidavit to attest to the truthfulness of the foregoing statements/facts and for whatever legal purposes it may serve.

A Special Power of Attorney is a form of Agency whereby a person binds himself to render specific/particular service or to do particular act/s in representation or on behalf of another (Principal), with the consent or authority of the latter. (Articles 1868 and 1876, Civil Code of the Philippines)

AFFIDAVIT OF QUIT CLAIM

            I, MAY BUKANGLIWAYWAY, Filipino, of legal age, married, with office address at Unit 70, 1228 Love Me Well Building, Wag Paasa, Quezon City, having been duly sworn in accordance with law, hereby depose and state that:

      1.      I am the President and authorized representative          of MABUHAY Corporation;    

2.      A Decision was rendered by the Regional Trial Court of Pasig City, Branch 35 on June 18, 2013 in a case entitled MABUHAY Corporation vs. Maginhawa Corporation and which became final and executory on January 18, 2014;

3.      A Writ of Execution dated 18 June 2014 was issued by the Regional Trial Court Branch 35 ordering  to seize goods and chattels of the defendants in the said case and make the sale thereof in accordance with law to satisfy the judgment in favor of Mabuhay Corporation;

4.      Whereas, the Board of Directors on September 28, 2014 approved the settlement of the loan granted to Maginhawa Corporation represented by its President Maganda Kho in the amount of FIVE MILLION PESOS (Php5,000,000.00);
  
5.      Whereas, Maganda Kho fully paid the amount of FIVE MILLION PESOS (Php5,000,000.00) on February 11, 2015 as the full  settlement of its loan obligation to MABUHAY Corporation; and

6.      That by virtue of the foregoing, I hereby voluntarily and knowingly quitclaim, release, discharge and/or waive any and all actions and rights subsisting, expected, real or apparent which MABUHAY Corporation may have against Maginhawa Corproation, its successors and assignees, its President, directors, officers and employees and hold them free from obligations, claims and other rights of action, monetary or otherwise, pertaining to any claim or action which I may have by virtue of my disinterest in the purchase of and waiver of whatever amount is due me over the purchase of the above-cited properties; and

7.      That I am executing this affidavit to attest to the truthfulness of the foregoing facts and for whatever legal purpose this may serve.
